When you register with the practice you will be asked to fill out a ‘Registration Form’ and a ‘New Patient Questionnaire’ and a ‘Vision On line – Patient Registration Form’. Then you will be given a new patient appointment with one of the GPs, so that a brief history of previous illnesses can be taken and routine tests can be carried out – blood pressure etc. This enables us to get some basic details about your health as it can take some time for the records to be sent to us from your previous Practice, particularly if this was outside of Scotland.
If you are on repeat medication or think you need a referral, please tell the receptionist so that a GP appointment can be arranged. After completing a form your repeat prescriptions will be set up on our system which will enable you to order them online.
This covers the area around Glenlivet, Ballindalloch and the surrounding villages. The Practice population is around 600 patients.

Patient Services Online Patient Registration Form
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
